Job Title: Evaluator
Location: Indianapolis, INDepartment: Behavioral Health Services
Employment Type: Full-Time
Shift: Monday – Friday, 8:00 A.M. – 5:00 P.M. (Schedule may vary based on program, client needs, and service setting.)
Job Summary:

As an Evaluator, you will conduct comprehensive screenings, assessments, and evaluations for individuals of all ages seeking behavioral health services. You will assess client needs, determine program eligibility and appropriateness, develop treatment recommendations, establish provisional diagnoses, and connect individuals with the services and resources that best meet their needs.
This role provides the opportunity to work collaboratively with clinical teams, community partners, clients, and families while supporting access to timely, appropriate, and person-centered care. Evaluators may also provide safety planning, brief therapeutic interventions, crisis support, and follow-up services as needed.

Key Responsibilities:
•
Conduct comprehensive screenings, assessments, and evaluations for individuals seeking behavioral health services.
•
Determine client eligibility, service needs, and appropriateness for available programs.
•
Gather and evaluate clinical, psychosocial, demographic, and other relevant information as part of the assessment process.
•
Establish provisional diagnoses and develop appropriate treatment recommendations based on assessment findings and applicable standards.
•
Develop individualized treatment plans and service recommendations as appropriate to the assigned program.
•
Provide safety planning and brief therapeutic interventions when clinically appropriate.
•
Make referrals to internal programs, community providers, and other resources based on client needs.
•
Provide or coordinate comprehensive follow-up plans for individuals following their assessment.
•
Participate in or coordinate emergency services and crisis intervention when necessary.
•
Maintain knowledge of available organizational programs, admission criteria, referral processes, and service requirements.
•
Maintain current knowledge of community resources and services available outside of the organization.
•
Maintain accurate, timely, and clinically appropriate documentation in the Electronic Health Record (EHR).
•
Complete required documentation and data collection activities within established timeframes.
•
Meet department-specific Collaborative Service Goals (CSGs), including established direct client care and productivity expectations.
•
Proactively coordinate services and follow-up to reduce missed appointments and unnecessary cancellations.
•
Participate in peer review staffing, clinical meetings, case consultations, staff development, and other required meetings.
•
Monitor and comply with Quality Assurance, Compliance, Revenue Cycle, and clinical documentation requirements.
•
Report critical incidents and staff responses to appropriate supervising leadership in accordance with organizational procedures.
•
Provide education and information through community presentations and public forums to promote mental health awareness and appropriate community response.
•
Support organizational initiatives involving data collection, program evaluation, grant applications, and other management needs.
•
Complete all required training, professional development, and performance evaluations.
•
Practice in accordance with organizational operational guidelines, professional standards, ethical requirements, and applicable regulations.
•
Perform additional duties and special projects as assigned by department leadership.
Education and/or Experience Qualifications:
•
Master’s degree in Social Work, Psychology, Counseling, or a related behavioral health field from an accredited institution required.
•
Previous experience conducting behavioral health assessments, screenings, or evaluations preferred.
•
Experience working with individuals across diverse age groups and presenting behavioral health needs preferred.
•
Experience in mental health and/or substance use disorder treatment settings preferred.
•
Experience with crisis intervention, safety planning, referrals, and care coordination preferred.
•
Experience with Electronic Health Records and clinical documentation preferred.
Required License/Certification:
•
Current, unrestricted professional licensure required based on the state of employment and applicable area of practice.
•
Licensure may include applicable clinical behavioral health credentials such as:
•
Licensed Clinical Social Worker (LCSW)
•
Licensed Professional Counselor (LPC)
•
Licensed Marriage and Family Therapist (LMFT)
•
Other equivalent professional licenses as permitted by applicable state regulations.
•
Must maintain all required professional licenses and certifications throughout employment.
•
Must provide documentation of applicable licenses and credentials.
•
Valid driver’s license, acceptable driving record, and current automobile insurance required.
Additional Qualifications:
•
Strong knowledge of behavioral health assessment, screening, and treatment planning practices.
•
Knowledge of mental health and/or substance use disorders and related treatment resources.
•
Ability to identify client needs and determine appropriate levels of care and referral options.
•
Strong understanding of professional ethics, clinical standards, and applicable regulatory requirements.
•
Excellent verbal and written communication skills.
•
Ability to communicate effectively with individuals from diverse backgrounds, including clients, families, providers, community partners, and other stakeholders.
•
Strong clinical judgment, critical-thinking, and problem-solving skills.
•
Ability to apply sound judgment when evaluating complex or sensitive situations.
•
Ability to maintain accurate and timely clinical documentation.
•
Strong organizational and time-management skills with the ability to manage competing priorities.
•
Ability to respond appropriately and professionally during crisis situations.
•
Ability to establish effective relationships through in-person, telephone, and telehealth interactions.
•
Knowledge of community-based resources and behavioral health service systems.
•
Ability to maintain confidentiality and appropriately safeguard protected health information.
•
Proficiency with Electronic Health Records, Microsoft Office, and other technology used to support clinical services.
•
Commitment to person-centered, trauma-informed, culturally responsive, and recovery-oriented care.
•
Ability to work independently while collaborating effectively with multidisciplinary teams.
•
Ability to accept direction, supervision, feedback, and coaching.
•
Ability to provide clinical supervision to trainees when approved, appropriately licensed, and qualified to do so.
Supervisory Requirements:
•
None.
Clinical supervision responsibilities may be assigned to appropriately licensed and qualified Evaluators based on program needs and applicable professional requirements.
Employment Requirements:
•
Successful completion of background screening, including criminal history, driving record, abuse/neglect, and fingerprint checks.
•
Completion of New Hire Orientation upon hire.
•
Completion of all required training, including Relias training, upon hire and annually thereafter.
•
Current driver’s license, acceptable driving record, and proof of automobile insurance.
•
Maintain unrestricted professional licensure appropriate to the location and area of practice.
•
Maintain all required professional certifications and credentials throughout employment.
•
Complete all department-specific training and competency requirements within established timeframes.
•
Comply with applicable state and federal regulations, accreditation standards, organizational policies, and program-specific requirements.
•
Complete required clinical documentation, quality assurance, compliance, and utilization review activities within established timeframes.
•
Participate in required supervision, staffing, meetings, professional development, and performance evaluation activities.
•
Additional program-specific requirements may apply based on assigned department, population, and service setting.
Physical Requirements:
•
Ability to perform sedentary work, including sitting for extended periods throughout the workday.
•
Frequent use of hands, fingers, and arms for typing, writing, clinical documentation, and Electronic Health Record management.
•
Ability to occasionally stand, walk, bend, reach, and lift materials weighing up to 10 pounds.
•
Ability to travel between office, community, and client locations as required by the assigned program.
•
Ability to work across office, clinical, community-based, and telehealth environments.
•
Ability to respond appropriately to crisis situations and follow established safety and intervention procedures.
•
Visual ability to review clinical documentation and work on computer screens for extended periods.
•
Ability to communicate effectively in person, by telephone, and through telehealth platforms.
•
