Job Title: Community Support Specialist
Location: Indianapolis, IN
Department: Behavioral Health Services
Employment Type: Full-Time
Shift: Monday – Friday, 8:00 A.M. – 5:00 P.M. (Schedule may vary based on program, client needs, and service setting.)

As a Community Support Specialist, you will provide community-based behavioral health services to children, adolescents, and/or adults while helping clients achieve their individualized treatment and recovery goals. You will use strengths-based, person-centered, and evidence-based approaches to help individuals develop coping skills, improve functioning, build resilience, and strengthen their support networks.
You will work closely with clients, families, therapists, case managers, healthcare providers, schools, and community organizations to coordinate services and ensure individuals have access to the resources they need to achieve greater stability and independence.

Key Responsibilities:
•
Provide community-based behavioral health services to children, adolescents, and/or adults based on individualized treatment goals.
•
Support clients in developing and maintaining skills necessary to function successfully in their homes, schools, workplaces, and communities.
•
Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment and service plans.
•
Use a strengths-based and recovery-focused approach to promote empowerment, independence, skill development, and resilience.
•
Implement evidence-based interventions, including Motivational Interviewing, Positive Reinforcement Techniques, Trauma-Informed Strategies, and skills informed by Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).
•
Provide behavioral and skill-building interventions designed to address identified mental health and/or substance use needs.
•
Teach and model healthy coping strategies, problem-solving skills, communication skills, and strategies for managing behavioral health symptoms.
•
Connect clients and families with appropriate community resources and natural supports.
•
Coordinate services with primary care providers, psychiatrists, therapists, case managers, schools, community organizations, and other professionals.
•
Develop and maintain positive working relationships with parents, caregivers, teachers, counselors, healthcare providers, and other individuals involved in the client’s care.
•
Support clients in achieving goals related to family, school, employment, community functioning, and overall independence.
•
Prepare for and facilitate Psychosocial Rehabilitation (PSR) group sessions when applicable to client treatment plan goals.
•
Maintain accurate, timely, and comprehensive documentation of client interactions, interventions, progress, and services provided.
•
Participate in treatment planning, team meetings, case consultations, staffing, and discharge planning activities.
•
Participate in staff development, training, and continuing education activities to maintain and enhance professional knowledge and skills.
•
Contribute to quality improvement initiatives, including data collection, analysis, reporting, and implementation of service improvements.
•
Manage multiple client needs and coordinate services across a variety of community resources while maintaining effective communication with internal and external partners.
•
Respond appropriately to crisis situations and follow established intervention and safety procedures.
•
ACT/ACT TAY/IMPART Programs: Demonstrate knowledge of the basic elements and theory of the applicable ACT model, core team processes, and services provided by ACT team members.
•
Maintain compliance with organizational policies, professional standards, and applicable state and federal regulations.
•
Perform additional duties as assigned by leadership.
Education and/or Experience Qualifications:
Candidates must meet one of the following qualifications:
•
Bachelor’s degree in Psychology, Social Work, or another specialized human services field; or
•
Bachelor’s degree in an unrelated field and two (2) years of related behavioral health experience; or
•
Any four-year combination of higher education and two (2) years of qualifying related work experience; or
•
Associate of Applied Science degree in Behavioral Health Support; or
•
Four (4) years of qualifying full-time experience in behavioral health or a related human services field.
Qualifying Experience:
Qualifying experience must include providing services to individuals with mental health disorders, substance use disorders, and/or developmental disabilities. Experience may include:
•
Providing individual or group services with a rehabilitation, habilitation, recovery, or resiliency focus.
•
Teaching and modeling coping strategies for individuals managing psychiatric, developmental, or substance use-related challenges.
•
Connecting individuals with natural and community-based resources and support systems.
•
Supporting individuals in obtaining and maintaining employment.
•
Supporting appropriate functioning within family, school, employment, and community environments.
•
Assisting individuals in achieving goals and objectives identified in individualized treatment plans.
Required License/Certification:
•
Valid driver’s license required.
•
Current automobile insurance required.
•
Acceptable driving record required.
•
Additional certifications or training may be required based on program assignment and applicable state regulations.
Additional Qualifications:
•
Knowledge of behavioral health case management and community support practices.
•
Understanding of rehabilitation, habilitation, recovery, and resiliency principles.
•
Knowledge of mental health and/or substance use disorders and available community resources.
•
Familiarity with behavioral health treatment approaches and evidence-based interventions.
•
Knowledge of applicable legal, ethical, and regulatory requirements related to behavioral health services.
•
Strong interpersonal and relationship-building skills.
•
Excellent verbal and written communication skills.
•
Ability to establish effective relationships with clients, families, providers, schools, and community partners.
•
Strong organizational and time-management skills.
•
Ability to manage multiple priorities and coordinate services across multiple agencies and resources.
•
Strong problem-solving and critical-thinking skills.
•
Ability to work independently while remaining accountable to supervision and organizational standards.
•
Ability to maintain accurate and timely clinical documentation.

•

Proficiency with Electronic Health Records and common office technology preferred.
Supervisory Requirements:
•

Employment Requirements:
•
Successful completion of background screening, including criminal history, driving record, abuse/neglect, and fingerprint checks.
•
Completion of New Hire Orientation upon hire.
•
Completion of all required training, including Relias training, upon hire and annually thereafter.
•
Current driver’s license, acceptable driving record, and proof of current automobile insurance.
•
Complete required professional development and training necessary to meet applicable DMH and program standards.
•
Maintain knowledge of the policies, procedures, regulations, and contractual requirements applicable to the assigned program.
•
Must be at least 21 years of age to operate a company-owned vehicle.
•
Must be at least 23 years of age to operate a company-owned 15-passenger van.
•
Must have reliable transportation to perform community-based responsibilities and, when required, transport clients in a personal vehicle.
•
Must comply with all applicable state, federal, organizational, contractual, and regulatory requirements.
•
Additional program-specific training and credentialing requirements may apply.
Physical Requirements:
•
Ability to perform sedentary work, including sitting for extended periods throughout the workday.
•
Frequent use of hands, fingers, and arms for typing, writing, documentation, and Electronic Health Record management.
•
Ability to occasionally stand, walk, bend, reach, and lift, carry, push, or pull objects weighing up to 10 pounds.
•
Ability to travel frequently within the community and between client, school, office, residential, and other service locations as required.
•
Ability to accompany and, when required, transport clients in accordance with organizational policies and applicable safety requirements.
•
Ability to work in a variety of environments, including offices, schools, homes, community settings, and other locations.
•
Ability to respond appropriately to crisis situations and follow established safety and behavioral intervention procedures.
•
Visual ability to read clinical documentation and work on computer screens for extended periods.
•
Ability to communicate effectively in person, by telephone, and through electronic/telehealth platforms as applicable.
•
