Provide Compassionate Spiritual Care
• Assess spiritual and emotional needs of patients, caregivers, and families
• Develop and support individualized spiritual care plans
• Provide spiritual counseling, emotional support, grief support, and compassionate presence
• Support patients and families through difficult conversations, transitions, and end-of-life experiences
Serve as a Collaborative Care Partner
• Participate in interdisciplinary team (IDT) meetings and care planning
• Collaborate closely with nurses, social workers, physicians, aides, and community clergy
• Help ensure holistic, patient-centered care aligned with patient goals and wishes
• Coordinate visits from local clergy or faith leaders when appropriate
Support Patients & Families
• Build meaningful, trusting relationships with patients and loved ones
• Provide comfort and guidance while respecting diverse faiths, beliefs, cultures, and lifestyles
• Help families navigate grief, anticipatory loss, and emotional stress with empathy and sensitivity
• Serve as a calming and supportive presence during deeply personal moments
Drive Quality & Documentation
• Maintain accurate and timely clinical documentation
• Participate in quality and compliance initiatives
• Support hospice philosophy and agency standards of care
• Maintain ongoing professional development and continuing education
