As an Inpatient Care Navigator Social Worker, you will:
• Patient Assessment: Conduct comprehensive assessments to identify patients’ needs, strengths, and barriers to care. Develop individualized care plans that promote recovery and address psychosocial factors.
• Care Coordination: Collaborate with psychiatrists, therapists, social workers, and other healthcare professionals to ensure integrated and effective treatment. Facilitate communication among team members to support continuity of care.
• Resource Connection: Identify and connect patients with appropriate community resources, including support groups, social services, and educational programs, to enhance their recovery journey.
• Patient and Family Support: Provide education and emotional support to patients and their families regarding treatment options, coping strategies, and available resources. Address any concerns or questions they may have.
• Monitoring and Follow-Up: Regularly monitor patient progress and adjust care plans as needed. Schedule follow-up appointments and ensure patients are adhering to treatment recommendations.
• Documentation: Maintain accurate and up-to-date records of patient interactions, care plans, and referrals in compliance with facility policies and regulatory requirements.
• Quality Improvement: Participate in quality assurance initiatives, gather data on patient outcomes, and contribute to the development of best practices for outpatient behavioral health care.
• Crisis Intervention: Respond effectively to crisis situations, providing support and stabilization for patients in distress.
