This role works closely with community partners and local stakeholders to ensure comprehensive, coordinated care. Case Managers meet with clients in a variety of settings, including the office, hospital, homes, schools, workplaces, and other community locations. Through case management services, resource coordination, and ongoing support, this position helps individuals and families improve stability, wellness, and overall quality of life. A variety of clinical training and professional development opportunities are available based on program assignment and individual interests.
This position is available across the following Centerstone Labelle Programs:
Community Action Treatment (CAT) Team
Serve at-risk families with children experiencing mental and behavioral health crises. This community-based program provides intensive support to help families stabilize, strengthen functioning, and connect with critical resources.
CORe/Outpatient
Support individuals receiving treatment for mental health and/or substance use disorders. Provide care coordination, resource linkage, and ongoing support to help clients achieve recovery goals and improve overall well-being.
