About the role
*This is a full-time position offering a Monday-Friday schedule and requires travel across the West/Central area
As a Licensed Practical Nurse (LPN), you’ll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Key Responsibilities
· Deliver compassionate, direct patient care in accordance with state regulations
· Implement care plans developed by the RN and document progress accurately
· Monitor patient conditions and communicate observations to the RN and physician
· Collaborate with the care team to ensure seamless, high-quality service
· Participate in professional development and team meetings
Requirements
